Form Instructions:
This form was created for those who would either prefer not
to use the "Buy It" buttons, would prefer to use one order form or
want to mail their payment into us. For your convenience, choose either
option:
1. Complete form, print out and mail to the
above address with your payment.
2. Complete form with credit card information
and submit for processing. You will receive a confirmation email
indicating when you items will ship. If purchasing more than one item,
shipping charges will be reduced.
3. For check and money order payments, complete
form and submit, them mail your payment to the above address. We will send
you a confirmation email when payment is received. If purchasing more than
one item, shipping charges will be reduced and this amount will be included in
your confirmation email. Please do not mail payment until you receive the
total amount.
PLEASE NOTE: If using option 1 and purchasing
more than one item, please email us
first to obtain a reduced shipping charge.
